Collection Instructions:

Gently invert the sample 5-6 times after collection.

Specimen Transport:

Processing Instructions: 

1. Allow the specimen to clot for 30 minutes.

2. Centrifuge specimen within 2 hours of collection.

3. After centrifugation, specimen tubes without a gel barrier should have the serum aliquoted to a false bottom container.

4. Keep serum refrigerated until testing can be performed.

Interpretive

The finding of a positive “whole” antibody is important in identifying patients sensitized to particular foods. Subsequent food allergen component testing can provide additional information useful for patient management. Component testing results may also be used by an allergist/specialist to determine if an oral food challenge is appropriate.

  • It may predict the likelihood of peanut sensitivity but may not be solely predictive of reactions or allergic response
  • 78% of patients sensitized to peanuts may not be at risk for a systemic reaction
Ara h 8 (component)
  • Lower risk of systemic reaction
  • Risk of mild, localized symptoms such as itching or tingling of the lips, mouth, and oropharynx
  • Cross-reactive with pollens, such as the birch tree
  • The patient likely is not restricted to peanut-free zones
Ara h 9 (component)
  • Variable risk of systemic reaction
  • Often accompanied by sensitization to other peanut proteins
  • Cross-reactive with fruits containing pits, such as peaches
  • There is patient variability regarding restrictions to peanut-free zones
Ara h 1, 2, 3, 6 (components)
  • Higher risk of systemic reactions, including anaphylaxis
  • Family, colleagues, and teachers should be aware of the patient's peanut allergy and have a plan. The prescription of epinephrine auto-injector should also be considered
  • Should choose peanut-free zones for the patient's safety
