Test Code LIPD or LAB18 Lipid Panel With Reflex Measured LDL, Serum or Plasma

Important Note

Note: Unless the order specifies "Lipid Panel Without Reflex Measured LDL," the measured LDL will be performed in cases where the triglyceride is >800 mg/dL. This is due to the inability to calculate an LDL if the triglyceride level is >800.

If a reflex-measured LDL is not desired, order LIPN (LAB2211)

Note about fasting requirements: Fasting is no longer required for lipid panels.  See the December 2020 LabWire (Resources section) for details.

There may be cases where a fasting triglyceride level is desired.  If so, the patient should be instructed to fast from food or beverages (other than water) for 12 hours before collecting a sample.  Fasting patients patient should be on a normal diet and maintain a stable weight for one week before testing.  Consuming alcohol or fatty foods up to 24 hours prior to specimen collection can also increase serum triglycerides.

*LDL is calculated by the NIH (Sampson) formula. If the triglycerides result is > 800 mg/dL, the LDL - calculated result will not be performed. Instead, a measured LDL is reflexed and performed.  See the LabWire 2020 article under "Resources" for additional information.

Optimal Collection Volume: 

4.5 mL; full tube

Specimen Minimum Volume:
  • Adults: 1 mL whole blood
  • Neonates: 1 microtainer (400-600 uL)
Collection Instructions:

Gently invert sample 5-6 times after collection.

Specimen Transport:

Refrigerate

Processing Instructions: 

1. If serum, allow the sample to clot for 30 minutes.

2. Centrifuge specimen within 2 hours of collection.

3. After centrifugation, specimen tubes without a gel barrier should have the serum or plasma aliquoted to a false bottom container.

4. Keep serum/plasma refrigerated until testing can be performed.
