This is a generic miscellaneous test request for courtesy sample collection and sample submission to third party laboratories.

For courtesy sample collections which are returned to patient or clinical staff for shipment: A specimen collection kit should be supplied to the phlebotomist. Phlebotomist should follow specimen collection requirements within the kit.

Tests which will require results back to Bronson Laboratory and the patient medical record must be approved per Bronson policy and ordered using Bronson Test code SOMIS.
