Specimen Requirements

Notify Pathology office at extension 8997 when specimen
is ready and provide patient’s full name, date of birth,
operating room (O.R.) number and surgeon’s name.

Specimen Type: Fresh tissue

Container/Tube: Plastic container with no
added fixative or preservative

Collection Instructions:

1. Specimen source is required.

2. Send specimen immediately.

3. Label container with patient’s full name and date of
birth.

4. Tube specimen with completed paperwork to tube station
117.

5. Cause for rejection includes unlabeled or incorrectly labeled
specimen.

Forms: Pathologist Consultation Form and
Pathology order form; indicate patient’s full name, date of
birth, date of service, ordering physician, and O.R.
room number.
