Specimen Type: Placenta

Container/Tube: Large plastic container
with lid with formalin

Specimen Volume: Entire placenta

Collection Instructions:

1. Submit entire placenta fixed in formalin.

2. Label container with patient’s full name and date of
birth.

3. Cause for rejection includes unlabeled or incorrectly labeled
specimen which will be returned for correct identification.

Forms:

1. Pathologist Consultation Form must be completed by
the physician; indicate patient’s full name, date of
birth, date of service, and pertinent clinical history.
