Test Code ACTH60 ACTH Stimulation Test, Baseline and 60-Minute Post Injection

Important Note

This test consists of a Baseline Cortisol (Pre Cosyntropin injection) and a 60 minute post injection Cortisol only. Requests for additional Cortisol levels, ACTH (Adrenocorticotropic Hormone) or other laboratory testing in unison with the stimulation testing must be submitted as a separate request and coordinated prior to injection to ensure proper draw times.

Specimen Requirements

All ACTH Stimulation Tests must be scheduled in advance.

A baseline sample is collected, then Cosyntropin is injected intramuscularly by an RN. A peak sample is collected at 60-minutes post injection.

 

Submit only 1 of the following specimens:

 

Specimen Type: Serum
Container/Tube: Gold-top serum gel-Plain red-top tube is also acceptable.
Specimen Volume: 5 mL

Specimen Minimum Volume: 0.5 mL

Collection Instructions: Include time of draw on specimen.

 

Specimen Type: Plasma

Container/Tube: Green top (heparin)
Specimen Volume: 5 mL

Specimen Minimum Volume: 0.5 mL

Collection Instructions: Include time of draw on specimen.

Reference Values

Normal ACTH stimulation interpretation is cortisol >15 ug/dL. 

NOTE: The usual normal response is an approximate doubling of the baseline level, provided the baseline level does not exceed the normal range (6.0-23.0 ug/dL). Many patients with normal adrenal function, however, do not respond to the expected degree.
